MWH Associate Wellness Works Wonders

Cardiopulmonary Health & Fitness (CPHF) is offering a Wellness Program for all Associates. Listed below is the information you will need to get started on your Wellness Program!

1. What do I have to do to get into the Wellness Program?
a. Come to the CPHF Department on the 2nd floor of the ASC, Suite 240.
b. Pick up your physician release and medical history form. 
c. Have your physician approve and sign your medical release.

2. Next you need to:
a. You may call between the hours of 9 am and 1 pm to register with the department’s Patient Access registrar.
b. Pay your $116 fee by cash or check. This fee covers your sub-max stress test, exercise prescription, and use of the exercise equipment the first calendar year.

3. What is the graded exercise test?
The test allows the exercise physiologist to assess your cardiovascular fitness level. From the information gathered from the test, such as heart rate response and blood pressure response, an individualized exercise prescription will be developed for you.

4. What is an exercise prescription?
The exercise prescription gives you a target heart rate range and workloads for exercise.

5. How do I learn to use the equipment?
After your test, you will be given a demonstration by the CPHF staff.

6. When may I exercise?

Mon.,Wed. & Fri.
Before 06:30am & after 5:00pm

Tues. & Thurs.
Around the Clock

Sat. & Sun.
Around the Clock

7. How long will it be before I may start exercising?
You need to return your medical history form and physician release form and set up an appointment for a sub-max stress test. This appointment will take 1 hour. As soon as you have been tested and given an exercise prescription, you may begin your exercise program.

8. How much will this cost me?
The initial year, the fee is $116 for the assessment and use of the equipment. If you do not allow your membership to lapse, each following year will then be an annual renewal fee of $59 on each yearly anniversary date based on your start date.
