VHVI Heart Failure Clinic Team

VHVI Heart Failure Clinic

VHVI’s Heart Failure Clinic has been developed to help patients with heart failure live better lives. We help patients understand the disease process, recognize early warning signs, and as a result, receive effective treatment sooner.

Patients are managed at the clinic by a licensed nurse practitioner with extensive experience in heart failure management. All care is supervised by a board certified cardiologist.

Our Heart Failure Clinic provides:

-Early follow-up after a patient has been discharged from the hospital
-Education for patients and loved ones to help manage the disease including easy to read educational material, daily weight log and other support tools
-Regular visits to monitor signs and symptoms associated with the disease
-Early recognition of problems, so they can be treated as an outpatient rather than in the hospital
-Management of heart failure medications
-Telephone consultations
-Coordination with each patient’s primary physician and/or cardiologist, plus referrals to other health care providers as needed

All protocols and care based on American College of Cardiology/American Heart Association Guidelines.

We are located at 2300 Fall Hill Avenue, Suite 234 Fredericksburg, VA 22401.

Hours of operation are Tuesday, Wednesday and Fridays from 8:00 a.m. - 4:00 p.m.
